Moments after her birth, Lua Di Felice underwent a surgery known as a frenectomy. The procedure involves cutting the membrane under the baby’s tongue (also known as the bridle). However, the operation divides opinion among Brazilian doctors.

Since 2014, the frequency of surgeries has been increasing. This is because the Ministry of Health has made the “linguina test” mandatory among newborns. The exam is performed in the first days of the child’s life and can diagnose tongue binding early.

However, the Brazilian Society of Pediatrics (SBP) is officially against the mandatory exam. The justification would be that the procedure would encourage a lot of unnecessary surgery. Furthermore, in many cases it is possible to correct the defect with adjustments in breastfeeding.

Children like Lua, who suffer from the condition, have a shorter frenulum than normal. This restricts tongue movement and interferes with breastfeeding. According to IGM Pediatric Dentistry pediatric dentist Dr. Ilana Marques, the defect can occur in the labial or lingual frenulum.

“In the case of the labial frenulum, frenectomy is necessary when it causes a gap between the central teeth (called diastema), when it makes it difficult to clean these central teeth or when it causes a lack of lip seal causing mouth breathing”, explains.

In the case of the lingual frenulum, it is necessary when the tongue has ankyloglossia (stuck tongue). “It can cause early weaning in infants due to difficulty breastfeeding, as well as choking, excessive drooling, mouth breathing, and selective feeding,” indicates an expert.
