American singer William Martin "Billy" Joel has undergone brain surgery to treat normal pressure hydrocephalus, reporting significant improvements in both his memory and balance.
The 77-year-old musician revealed on his SiriusXM channel that surgeons implanted a brain shunt to relieve pressure caused by fluid accumulating inside his skull.
Joel explained that he had suffered from severe balance issues prior to the procedure, describing the sensation as feeling like he was swaying on a boat. He initially opposed undergoing brain surgery following his diagnosis in 2025, but he later decided to proceed with the operation so he would not lose his mind.
Normal pressure hydrocephalus is a neurological disorder in which excess cerebrospinal fluid builds up in the ventricles of the brain, exerting pressure on brain tissue. The condition commonly affects older adults and causes difficulty with movement, balance problems, and memory impairment. Surgical treatment typically involves placing a shunt, a small tube that diverts excess fluid away from the brain to be safely absorbed elsewhere in the body.

The Bronx-born hitmaker first announced in May 2025 that he had been diagnosed with normal pressure hydrocephalus. He noted that the physical demands of live concerts had aggravated the condition, leading to noticeable issues with his vision, hearing, and balance while performing on stage.
Joel explained that playing loud rock and roll music during his shows produced physical stress on his body similar to suffering a concussion every time he performed. Doctors initially prescribed physical therapy and recommended that he take a break from live shows until the symptoms caused by the disorder could be properly controlled.
