The former participant of Power couple Clara Maia gave birth to the twin children, the result of the marriage with André Coelho, with only 27 weeks of gestation because children were diagnosed with an acute fetal transfusion syndrome. Tulio did not survive and Theo is hospitalized in the hospital in the neonatal intensive care.

Acute fetal transfusion syndrome It is a complication that occurs from 10% to 15% of the identical twin pregnancies, which share the same placenta. It occurs when there is an unequal blood exchange between children through connections in the blood vessels of the placenta. The child called a receiver receives more blood and the call of a donor receives less, which causes complications for both fetuses.
Acute syndrome, like Clara Maia’s children, is considered a rarer and more sudden form than this syndrome. “It can suddenly happen, usually at the end of pregnancy or even during childbirth. In this situation, a child quickly loses blood to the other. This can cause great differences in the blood of the two to birth, which require immediate care by the Neonatology team”, explains the obstetric gynecologist of the Rafaela Brito house.
In case of fetal transfusion syndrome, both fetuses suffer, but in different ways. The donor child can suffer from the lack of blood and nutrients, becoming smaller. The receiver can have excess blood, which overload the heart. In the case of Clara, Tulio was the receiver and did not survive because of the heart overloaded by excess blood. Theo was the donor and was born smaller than his brother, with only 680 grams.
Diagnosis
According to the obstetrician, the diagnosis of the syndrome is made up of ultrasound tests, which show a child with a lot of liquid in the bag and another with little and also a difference in size with each other. “Therefore, in the pregnancies of the twins who share the placenta, it is recommended to make frequent ultrasound, usually every two weeks, from the sixteenth week of gestation.”
The main treatment in these cases is laser surgery that interrupts abnormal vascular connections in the placenta, but Clara has not had time to undergo this procedure. “This significantly increases the possibility of survival of children. In lighter cases, you can only follow ultrasounds closely. In some places, when the laser is not available, it can be removed from the excess liquid of the receptor child to relieve symptoms, but this is a temporary measure,” says Rafaela Brito.

Childbirth
The pediatrician Patrícia Consorte explains that the fetal-fetal transfusion syndrome is often associated with premature births, including the first gestational ages, such as Clara Maia. Premature deliveries are often made to preserve children’s lives.
