When hospital care is no longer needed, but the patient still needs follow-up, home care is an alternative. The process of dehospitalization (transfer of the patient from a hospital unit to home care) arises, in most cases, from hospitalization.

At home, the patient receives care similar to that in the hospital. “In the home environment and with the follow-up of a multidisciplinary team, we administer the drugs. In addition, we perform the medications, follow-up of patients undergoing mechanical ventilation and care for other clinical conditions,” says Flávia Canuto, Head commercial to the home doctor.

To start home care, the patient must meet 3 criteria. The first is clinical, i.e. having a medical indication and being clinically stable. There are also social criteria, such as having a 24-hour carer and home conditions, and logistical (allowing access by the multidisciplinary team to the house).

Benefits of home care

One of the great advantages that home care offers the patient is the return to the family environment, Flávia points out. “Our experience shows that in the comfort of home, feeling more emotionally secure, close to family and friends, the patient’s recovery is faster in most cases,” he says.

According to her, another big advantage is that continued treatment at home greatly reduces the risk of the patient catching infections. “There is also a lower risk of clinical complications and a decrease in unnecessary hospitalizations,” she points out.

Furthermore, home care also benefits hospitals. This is the case, for example, of the reduction of hospitalization for chronic and stable patients, thus freeing up beds for complex cases.
