This rare reaction occurred after consuming mushrooms, causing so-called shiitake dermatitis, a rare but intriguing condition that can affect individuals after consuming raw or undercooked shiitake mushrooms.

Symptoms of shiitake dermatitis

The case of the man who ended up in the emergency room with a rash on his back after consuming undercooked shiitake mushrooms is a vivid example of this condition. The painful rash was intensely itchy and covered his back, giving the appearance of being whipped in that area and seriously affecting his quality of life.




Causes of dermatitis

The doctors, after considering various diagnoses, came to the conclusion that it was shiitake dermatitis. This skin reaction is a response to the carbohydrate called lentinan found in these edible mushrooms.

This carbohydrate triggers the release of chemical messengers, causing inflammation and the development of the characteristic rash, which typically appears two to three days after consumption. It is important to note that lentinan is broken down when mushrooms are properly cooked at temperatures above 145°C, making them safe for consumption.

The global reach of a rare condition

Initially reported in Japan in 1977, shiitake dermatitis is not limited to Asia, where the mushrooms are commonly consumed. Therefore, although the condition is more common in regions where the consumption of this delicacy is a regular practice, some cases have been documented in various parts of the world, including Europe, North America and South America.

Treatment and prognosis

Generally, shiitake dermatitis resolves on its own without the need for specific treatment. However, patients may be given anti-inflammatory drugs, such as steroids and antihistamines, to relieve symptoms.

In the case of the 72-year-old man, he was given steroids to apply to his back and antihistamine tablets to be administered orally. After two weeks, the itching subsided, but the dark spots on the skin persisted, a common phenomenon after the skin inflammation subsides.

This episode serves as a reminder of the potential dangers that can arise when we consume seemingly harmless foods without proper preparation. Although the condition usually resolves on its own, it can be quite uncomfortable and painful. To avoid this reaction, correct preparation of shiitake mushrooms is essential, with cooking above 145°C. Therefore, when enjoying this delicacy, remember to cook it properly to avoid an unpleasant experience.
