Runner dies after peeing black; understand rhabdomyolysis

The nephrologist explains how intense physical activity can lead to this serious pathology

Runner Dorgival Celerino, 50, died last Wednesday (24), after feeling ill after a run. The event took place in Caruaru, in the Agreste region of Pernambuco. According to medical information, the man died from complications related to rhabdomyolysis.

The syndrome occurs after training and intense physical activity. Every time the body moves, the muscle “breaks down” and this breakdown generates enzymes that are metabolized by the liver and kidneys. Rhabdomyolysis occurs when the lesion is so large and generates the substance in an amount that the body is not prepared to absorb.

On Earth, Dr. Lucas Filogonio, nephrologist at the FelĂcio Rocho Hospital, explains that the difference between muscle pain resulting from common physical exercise and rhabdomyolysis is that “the former is tolerable and of low intensity while the latter is disabling and generally accompanied by difficulty in moving the affected limb”.

Information says that Dorgival Celerino participated in the race on Sunday (20th), did not feel well and was hospitalized. He would have presented black urine, another symptom of rhabdomyolysis. This happens because urine “acquires a dark color due to the deposition of heme pigments in the kidney resulting from the breakdown of muscle myoglobin. These heme pigments cause obstruction and damage to the renal tubules, resulting in acute kidney injury associated with rhabdomyolysis,” Lucas explains. Filogonius.

According to the doctor, to avoid the disorder, it is important to stay hydrated, avoid activities in situations of high heat or humidity and also avoid exercises that are disproportionate to the person’s physical capacity.

Another case of rhabdomyolysis also attracted media attention in 2023, when a 25-year-old woman ended up in intensive care after a spinning class.

According to the nephrologist, these two deaths caused a stir due to their announcement, but normally in Brazilian hospitals many cases of rhabdomyolysis are recorded every day, generally caused by trauma, drugs and intense physical exercise.
