Medicine acts as a blocking of hypochretin, neurotransmitter that regulates the warning status
THE National Health Surveillance Agency (Anvisa) approved, in April of this year, a new remedy for the treatment of insomnia. Called Remelating, the product was recorded with the commercial name Dayvigo and the doctors’ expectation is that it reaches the national market in the coming months.
The new drug and Eszopiclone, an anvisa invoiced drug in 2019, were considered the most effective remedies for the treatment of adult disorder in a destination -analysis conducted by Oxford researchers e Published in the magazine The hand in 2022.
The study evaluated the research on 36 drugs used to combat insomnia, considering the quality of sleep Participants, the effects of several treatments, the presence of adverse effects and the consequences of the interruption of therapy. Together, the tests amounted to 44,000 people.
How does medicine work?
Luciana Palombini, asleep doctor and member of the Brazilian Sleep Academy (ABS), explains that the memory belongs to a new class of medicines called hypochretin or hourly antagonists, a neurotransmitter involved in the state of waking and warning. In the body, the drug acts as a blocker, reducing the action of hypocretin.
According to Luciana, the initial studies suggest that the drug has Lower risk of causing addictionUnlike like this “Z drugs”, such as zolpidem and lameness, which are among the most consumed in Brazil in the treatment of insomnia.
Zy drugs act in Gaba-A, a type of receiver for neurotransmitter Gaba, the main inhibitor of the central nervous system. They were developed for use for short periods and low dosage, but some people have started to take these drugs recreationally.
“The Z drugs also act on receivers that help improve the well -being and feeling of stress And for this reason, many people used these drugs during the day and not at night, to sleep. With this, the risk of dependence grows even more, “he comments.
Pedro Genta, sleep doctor and collaborator professor at the Faculty of Medicine of University of San Paolo (FMUSP), however, recalls that when the Z drugs emerged, they also had the promise of having a low risk of dependence.
For him, despite the positive signs, it will only be possible that the memory has a lower risk of addiction in a few years, when the drug is used in a wider way.
What are the contraindications?
The flyer reports that the recommended dose is 5 mg, administered only once at night, a few minutes before the patient went to bed. Ingestion should occur at least 7 hours before the time scheduled to wake up and, depending on the clinical response and tolerability, the dose can be increased up to 10 mg.
For generation, use should not be continuous. “When there is an activity the next day that generates a little anxiety and can precipitate a more intense night of insomnia, (The patient) It is possible to use drugs in a more punctual way, which reduces the risk of dependence. “
The main adverse reactions associated with the use of the drug, with an incidence greater than 2% in clinical studies, are sleepy and fatigue. Sleep paralysis has also been identified, but with less incidence.
Medicine is contraindicated for patients with narcolepsy, sleep disorder characterized by excessive sleepiness during the day, even when the person slept well at night. In addition, it is not recommended for patients with severe hepatic insufficiency, children and adolescents.
How much does it cost?
Although the memory has been approved by Anvisa, it must have the maximum amount of sales approved by the regulation chamber of the medicinal market (CMED), which has not happened so far.
The decision of when it is marketed comes from the company in possession of the registration, the Japanese pharmacist Eisai, who has not yet released the release date.
Medicines are not the first option
Pharmacological treatment is not the first option in the fight against insomnia. “The first approach, and this is international consensus, is Cognitive Behavioral Therapy for Insomnia (TCC-I),” says Luciana.
“Those who have a complaints of insomnia usually have inadequate night habits, such as excessive screens or a lifestyle that maintains the problem. Behavioral treatment tries to identify these behaviors and adapt them together with the routine of the person,” he adds.
When the only treatment does not bring the expected results, it is possible to carry out the prescription of medicines. The ideal, however, is that they will be used temporarily, always with professional accompaniment and in combination with TCC-I.
