The image can be resolved spontaneously, but when this does not occur, surgery is required
The hospitalizations due to redundant foreskin, phimosis and paraphimosis had an increase of 81.58% among the boys from 10 to 19 years, according to the data of Ministry of Health Raised by the Brazilian company of Urology (SBU). The numbers went from 10,677 in 2015 to 19,387 in 2024.
According to Alexandre Cavalcante, urologist and coordinator of the Urology Department of the Syrian-Tabano hospital in Brasilia, there are two possible reasons to increase the registers. The first would be the late diagnosis, caused by the absence of consultations adjust with experts. “This should be reviewed, since this condition must be recognized and treated previously, avoiding serious problems,” he underlines.
“A second possibility would be the opposite. Children and teenagers with this condition that once they had not sought medical and hospital help, today they would have more access to public health and awareness in the care of their health,” he adds.
What is redundant preparation?
In the redundant foreskin, there is an excess of skin that covers the male genital organ, called foreskin. The image does not prevent the complete retraction to expose the penis head, called glans, but can make hygiene difficult, to encourage infections.
What is phimosis?
Fimosis is a condition in which man cannot withdraw the foreskin and expose the glans. Unlike the redundant foreskin, excess skin makes it really difficult or preventing the exposure of the penis head.
Fimosis can be classified in the primary and secondary. The primary, also called physiological, is a current condition from birth, due to the natural adherence of the foreskin to the glans. The secondary arises after birth, usually caused by infections, inflammation or trauma in the foreskin, which lead to the narrowing of the skin and make the retreat difficult.
According to Cavalcante, the picture can be solved spontaneously during the development of the child. But in situations where the spontaneous opening of the foreskin does not occur, nor in adults or children, surgical treatment is needed.
What can phimosis cause?
According to Daniel Zylbersztejn, coordinator of the Department of Teenage Urology of the Brazilian Society of Urology (SBU), the phimosis can make it difficult for the individual Peni correctly sanitized.
The difficulty of cleaning the area, in turn, can generate a bad smell, increase the risk of urinary tract infection, encourage inflammation (called Banite or Balanopostitis) and increase the risk of Penis cancer.
Another possible consequence of the condition is the paraffinosis. “It happens when the foreskin strangles the head of the penis and can no longer return to the original position. This causes penis ischemia, as well as a very strong pain. In such cases, it is necessary to go to the hospital to undergo an urgent surgery or a maneuver to return with the pre -empty”, detail.
Does phimosis reduce fertility?
It is not difficult to find comments on social networks by saying that phimosis reduces fertility. Zylbersztejn, however, states that the condition does not cause problems in this regard. It can generate discomfort during sexual intercourse, but does not affect the reproductive capacity of men.
The condition does not even cause impotence. “Depending on the degree, there may be a discomfort because the glans remains blocked in the foreskin, generating greater friction, but does not damage the erection,” says the Ministry of Health.
Circumcision
The procedure indicated for cases of phimosis is the postectomy, also known as circumcision. According to Cavalcante, it is performed under local anesthesia and can be accompanied by sedation in adolescent and adult patients. In children, general anesthesia is usually chosen. The patient is discharged immediately after surgery and should follow specific care, such as the delicate cleaning of the region and daily application ointments that help in healing.
“The first few days are generally more uncomfortable due to the sensitivity of the region made, but it is usually easily postoperative surgery. To avoid problems after surgery, we recommend the patient not to carry out physical activity within the first 10 days or more if physical activity has an impact,” he says.
In the case of sexual relationships, we indicate to wait for the complete healing of the wound. “Above all, in case of doubt about the evolution of the healing of wounds in the postoperative period, the patient is looking for the doctor or health service in which he suffered surgery for revaluation,” says Cavalcante.
Teenagers and urologists
To encourage children and teenagers to seek help for these and other conditions, SBU develops this month the eighth edition of the #Mamprouro campaign, focused on the younger audience.
“The campaign underlines that male health should be careful soon, preventing future complications and promoting early diagnosis,” says Zylbersztejn, who is also coordinator of the campaign. “The goal is to demonstrate to the company that even children and teenagers must go to the doctor, whether urologist, pediatrician or family doctor.”
According to data from the Ministry of Health, in 2022, the frequency of girls from 12 to 19 years in medical treatment was almost 2.5 times higher than the boys of the same age group. The discrepancy was even more evident in specific consultations, with the girls aged between 12 and 18 who went to the gynecologist 18 times more than the boys looking for the urologist.
“The boys do not have an evident milestone as the beginning of menstruation for girls. Therefore, the habit of going to the doctor in advance,” says Zylbersztejn.
On social networks, through the @PortaldaurologyThe campaign brings together posts, videos and lives that clarify myths and deal with topics such as HPV vaccinationvaricocele, testicular twist, Premature ejaculation and penis size.
