But, being an early version, the serum caused a side effect on the Red Skull, which gets its name for that, as his face was severely deformed. At the end of the film, he takes the Tesseract and disappears during the final fight with Cap.

While the title points to possible Red Skull involvement, Marvel Studios is unlikely to make a direct adaptation of the comics.
