“In addition to standardized results, harmonization procedures with injectables have some disadvantages. First of all, the result is not definitive and requires constant application; the procedure, depending on the professional who injects, can cause facial distortions – as we have seen in recent years; It also presents important risks, such as skin necrosis and even blindness. But it is possible to improve the harmony and profile of the face without needles, with natural and definitive results with the combination of techniques,” he explains. “Usually, rhinoplasty and genioplasty, combined, are responsible for improving the harmony and profile of the face. This association is called filoplasty,” adds the doctor.
Changing nasal aesthetics
According to Paolo, rhinoplasty is a complex procedure that alters nasal aesthetics. It can last from two to four hours and is carried out under general anesthesia, rhinoplasty can be performed in two ways: open or closed, both techniques involve incisions in the internal region of the nose.
“In open rhinoplasty, an external incision is also made in the region of the columella, the tissue that divides the nostrils. In this way the surgeon is able to lift the skin of the nose so that the internal structures become more visible and manipulable. This technique is mainly used when major changes to the tip of the nose are needed, as it makes it easier to visualize the structures to be corrected. Open rhinoplasty leaves an external scar due to the incision of the columella, but this becomes imperceptible after a few months,” explains the specialist.
“In closed rhinoplasty, all cuts are made inside the nose and it is through these internal incisions that the surgeon sculpts and shapes the tissues. The advantage of this approach is that the procedure is quicker and leaves no visible scars. However, there is less visibility of the structures of the nose.”
Choice of the appropriate technique
The decision on which technique to use depends on the complexity of the surgery and the surgeon’s preferences. In the case of mentoplasty it is generally cited as a definitive alternative for harmonizing the face, especially in the case of additive mentoplasty, which increases the size of the chin making it more projected forward to make the face more harmonious.
“Surgery promotes this outcome through the use of implants or repositioning of the bone. The need for surgery is assessed through an objective examination and photos of the patient from the front and in profile,” explains the doctor. “The bone advancement surgery is performed under general anesthesia and lasts approximately 1 hour,” comments the plastic surgeon.
The position of the incision can also vary: either it is made inside the mouth, called intraoral, in the lower gingivolabial sulcus, or under the chin, called submental. Next, the jaw bone is moved, cutting the chin bone to reposition it with the help of a small titanium plate, providing a more natural result and using the patient’s bone structure.
“Finally, with the chin position properly fixed, the surgical incision is closed. We recommend using a specific band to reduce swelling in the area in the first few days,” explains Paolo.
Combination of procedures
By combining the two procedures, filoplasty can correct the disproportion between the chin and nose in a single operation, thus helping to significantly improve the harmony of the face and, consequently, the patient’s self-esteem and confidence, since the nose and chin are areas fundamental for facial aesthetics.
“In this procedure, the genioplasty is generally performed first and then the rhinoplasty. The process is performed under general anesthesia and lasts, on average, 5 hours.”
As for the results, which are definitive, they appear after the swelling that appears after the surgery has reduced, which takes three to six months, according to Paolo Rubez. “The postoperative period is the same as that of most surgical operations, requiring the patient to rest for two weeks and not engage in physical exercise,” he concludes.
