Hair loss is a phenomenon that affects millions of people around the world, regardless of gender or age. Hair loss not only impacts appearance, but can also have significant emotional and psychological repercussions.
Hevelyn Mendes, dermatologist specializing in trichology, explains that the main cause of hair loss is androgenetic alopecia, popularly known as male or female pattern baldness. This condition is hereditary and results from the sensitivity of hair follicles to dihydrotestosterone (DHT), a derivative of testosterone. In men it typically manifests itself with the receding of the frontal line and the rarefaction of the top of the head. In women, diffuse thinning of hair is observed, generally preserving the frontal hairline.
In addition to androgenetic alopecia, other factors can contribute to hair loss, including autoimmune diseases such as alopecia areata, thyroid disorders, nutritional deficiencies and even stress, which can trigger a condition called telogen effluvium, an alteration of the of hair growth leading to severe hair loss.
Understanding the underlying cause of hair loss is critical to determining the most effective treatment. For accurate diagnoses we use methods such as scalp dermoscopy and specific laboratory tests.
As for treatments, several options are available:
• Minoxidil: This topical medication is one of the most convenient and widely used treatments for hair loss. It works by stimulating hair follicles, prolonging the hair growth phase and increasing the size of follicles that have shrunk due to androgenetic alopecia. Minoxidil is available in concentrations, generally 5%, and can be used by both men and women. The oral dose will vary based on gender and level of baldness. Results vary, but most users can expect some hair regrowth or a significant decrease in the rate of hair loss after a few months of continuous use.
• Finasteride and Dutasteride: These drugs are inhibitors of 5-alpha-reductase, the enzyme responsible for converting testosterone to dihydrotestosterone (DHT). By reducing DHT levels, finasteride and dutasteride may help prevent continued hair loss and promote some regrowth in men with androgenetic alopecia. It is important to note that these drugs are not approved for women of childbearing age due to the risk of birth defects.
• Hair transplant: This surgical procedure involves removing hair follicles from areas of the scalp where hair is still plentiful (such as the back of the head) and replanting them into bald or thinning areas. There is one main technique: follicular unit extraction (FUE), which involves the removal of individual follicles.
• Low level laser therapy: This therapy uses lasers or LEDs to emit low-energy lights that stimulate hair growth by increasing blood circulation and metabolic activity in the hair follicles. LLLT devices include laser combs, helmets and bandanas. They are generally safe and can be used in combination with other hair loss treatments.
• Nutritional supplements: In cases where hair loss is due to nutritional deficiencies, supplements can be effective. The most common include iron, zinc, B vitamins (especially biotin), and vitamin D. These supplements help strengthen hair follicles and promote a healthier environment for hair growth.
In addition to these approaches, lifestyle changes, such as a balanced diet, stress management and proper hair care, are essential for hair health.
“I encourage my patients to openly discuss their concerns and expectations, allowing us to work together to find the best solution to their hair loss problems. Research continues to advance and, as a result, new therapies are always in development, promising future innovations in the treatment of hair loss and baldness,” concludes Hevelyn Mendes.
