For years, the dream of many women was to have large breasts, a goal easily achieved through breast augmentation with silicone implants in the region. But nowadays naturalness is at the center of all aesthetic procedures, including those focused on the appearance of the breast.
Therefore, while new patients are opting for smaller silicone implants, those who already had larger implants are choosing to replace them with smaller implants or even remove them. Carolina Dieckmann, Giovanna Antonelli, Manu Gavassi and Rafa Brittes are examples of people who have already changed or removed their silicone implants. But in many cases, simply removing the implant or replacing it with a smaller implant is not enough.
“When we perform a silicone prosthesis, the goal is to increase the volume of the breast. So, when we remove the prosthesis, a procedure called breast explant, there is a reduction in this volume and therefore, in addition to the prosthesis, the skin must also be treated. This is because this skin is adapted to that specific volume. So, when this volume is removed and there is no retraction of the skin, the region becomes flaccid. The doctor explains that the approach to the procedure will depend on each case, taking into account factors such as the size of the original implant, the size of the breast gland, and the patient’s choice between removing the implant or replacing it with a smaller one.
“If the patient who wants to reduce the size of her breasts has a good amount of mammary gland, the implant can be removed or replaced with a smaller size, but it will also be necessary to remove the skin to leave the beautiful shape, so scars will be inevitable, for example, in the shape of an inverted T. This procedure is called mastopexy,” says the doctor.
Small amounts
If the amount of mammary gland is very small, that is, if the breast was very small before the first surgery, a smaller prosthesis is necessary to give shape and volume to the breast.
“It is possible to remove the implant if the patient wishes, but this must be carefully discussed with the doctor to align the patient’s expectations regarding the final result. It is worth remembering that the more skin that is removed to shape the breast, the more significant the scarring that will result,” says the doctor.
He says that another option, in addition to implanting a smaller prosthesis, is to fill the breast with a fat graft taken from the patient herself through liposuction. “But the effect is not as predictable as with the prosthesis and several surgeries may be necessary to achieve the desired result,” adds the specialist.
Beatriz emphasizes that removing the prosthesis is simple, but shaping the new breast can take a long time, depending on the case. “However, remember that the removal or replacement of a prosthesis is a surgical procedure like any other, so pre- and post-operative care must be followed scrupulously,” recommends the plastic surgeon.
She says that the care for surgery depends on each case. “The more surgery you need to reshape your breast after implant removal or replacement, the more perioperative care you will need,” she adds.
Finally, the doctor explains that patient dissatisfaction is not the only reason why patients may choose to change silicone implants. “Statistics show that, after about 10 years, the possibility of problems with the implants increases, such as rupture or capsular contracture, which is the hardening of a capsule that the body forms around the implants, making them hard and even painful. Nothing may happen, but for this reason it is advisable to change the implant every 10 to 15 years,” explains the doctor.
However, if the patient is satisfied with the size and shape of her breast and needs to change the prosthesis due to problems such as capsular contracture, the procedure can be performed using the same scar as the first procedure. “In the case of capsular contracture, the capsule can be removed or incised, to make it more malleable, and a new prosthesis of the same size is placed, without having to remove the skin,” concludes Beatriz Lassance.
